  1. Before the meeting

    Assess decision-making capacity. Identify risks, benefits, and alternatives. Confirm advance directives and gather decision-makers.

  2. During the meeting

    Disclose options in plain language. Use teach-back to confirm understanding. Document a voluntary decision.

  3. After the meeting

    Sign and file the form in the chart. Provide a copy to the patient or representative. Document the conversation per facility policy.
